Modernizing Home Care Delivery

Our PC government is taking another step forward in strengthening home and community care with a new provincial technology platform designed to better connect the home care sector and support front-line staff—so Nova Scotians can receive quality care, faster.

The new platform will link all publicly funded home care agencies with Nova Scotia Health’s Continuing Care program, improving information sharing and coordination across the system. This will help ensure smoother transitions from hospital to home, reduce administrative burden, and allow care providers to spend more time where it matters most: with patients.

“Our government has made significant progress across healthcare. Today’s announcement is another step forward to improve home care for the tens of thousands of Nova Scotians who rely on it. Already, the number of people waiting for home care and staff vacancies are at record lows,” said Barbara Adams, Minister of Seniors and Long-Term Care. “We are building on our progress with this new platform, which will improve co-ordination, support better transitions home from hospital, and reduce paperwork and administrative burden. This will make sure staff have the tools they need to make their job better.”

Expected to go live in mid-2027, the platform will support faster and more efficient scheduling. People receiving care—and their families—will be able to view upcoming appointments, while real-time updates will ensure cancelled visits are quickly rebooked, improving efficiency and increasing access to care.

The platform will be designed and implemented by AlayaCare Inc., a Canadian company specializing in cloud-based home and community care solutions. The investment totals approximately $19 million over five years.

With about 40,000 Nova Scotians relying on provincial home care programs, this investment reflects our government’s commitment under Action for Health to modernize healthcare delivery and expand access to care through smart, practical technology.
