New Citadel Health Home to Bring Primary Care to Thousands

Families in Halifax and surrounding communities now have better access to primary care with the opening of the new Citadel Health Home by our PC government.

The health home is expected to be fully operational by October and is part of a broader effort to reduce the number of Nova Scotians waiting on the Need a Family Practice Registry. The Citadel health home is expected to take on more than 6,000 patients once it is fully operational.

The health home will be staffed by a collaborative care team that includes four family physicians, one nurse practitioner, three family practice nurses, and one licensed practical nurse. Over time, the team will also be supported by clerical staff and allied health professionals such as social workers, dietitians, and physiotherapists—ensuring patients receive well-rounded care under one roof

“We continue to build and strengthen health homes across the province so patients have access to comprehensive primary healthcare in their communities,” said Adegoke Fadare, MLA for Clayton Park West, on behalf of Health and Wellness Minister Michelle Thompson. “These health homes will have a significant impact on further reducing the number of people waiting on the Need a Family Practice Registry.”

In time, the health home will also offer clerical support and allied health professionals such as social workers, physiotherapists, or dietitians—ensuring patients receive the right care, when and where they need it.The Citadel Health Home is one of three new health homes being established in the Halifax Regional Municipality, alongside Hobsons Lake and Needham. It’s part of our Houston government’s Action for Health plan to improve access to care and build a stronger healthcare system for all Nova Scotians.
